hereby have chosen to provide transportation for this child TO and FROM all extracurricular

activities or events for which District transportation is not provided.

I understand that by signing this waiver, I take full responsibility for the transportation of this

child TO and FROM all extracurricular activities and events during the 20___/20___ school year

for which District transportation is not provided. I also understand that this waiver releases the

District from any and all liability or claims regarding the transportation of this child TO and

FROM extracurricular activities or events for which District transportation is not provided.
